School / Youth Group / Youth Ministry Request Form
This form is to request a workshop, speaker, candy bag assembly event drive, or sign-up for a monthly walk for your group, ministry, or school. We have a number of individuals who are great speakers and would be pleased to speak to your group and perhaps have a brief workshop and candy assembly event. The following information will assist in our choosing an appropriate speaker for your event.
